'Miss' on D1 Form (DL Application Form)

I know this is the 1000th post about the 'miss' title preceding names on the UK driving licence.
Linked below is a D1 application form, and I cannot find the part that requests the removal of 'Miss', perhaps this is just ticking the 'Ms' box but I doubt many teenage girls tick that, and I can't work out why g0d has such trouble understanding the concept.
